Consent to the Collection and
Use of Your Personal Information
Our company, 9385-8363 QUÉBEC INC., is a Canadian corporation incorporated in Québec, doing business under the trade name “CreditBook,” with its head office located at 1641A, 440 West, Laval, Québec, H7L 4J5 (hereinafter referred to as “CreditBook”).
The purpose of this document is to request and obtain your consent and to provide transparency regarding our personal information processing practices. You may withdraw your consent at any time, subject to legal or contractual restrictions.
CreditBook provides this form to its clients (the “Clients” or, individually, a “Client”) to enable them to determine your eligibility for [“subscription services for financial education plans” or “a loan”] (the “Service”). CreditBook may also provide its Clients with services or products designed to facilitate the management of their relationship with you and the Services.
Once the form has been submitted, the Client will collect and process the personal information about you contained in the form, including sensitive information relating to your financial situation and credit history, for the purpose of determining your eligibility for the Service and for any other legitimate purpose of the Client. This information may be disclosed to us in our capacity as a service provider and may also be used to create a file about you.
Where applicable, we may use and disclose your personal information independently of the Client in order to:
create a file about you;
prepare reports, scores or alerts relating to your creditworthiness or debt profile and share them with our Clients or contractual partners in connection with related services;
if this feature is enabled by the Client, create a Credit Boost user account for you, allowing you to track your application and its processing in real time;
contact you when required, for example regarding your Credit Boost account; and
fulfill our legal and contractual obligations and enforce our rights.
The information used for these purposes may include the information you provide through this form, information we may infer from it, and information obtained from third parties, such as financial institutions or other Clients, with your consent.
You have the right to access and correct the personal information we hold about you.

In addition, we ask that you consent to our collection, use and potential disclosure of your personal information, including, where applicable, any detailed financial information you may provide to us, as described below. Please indicate whether you consent to each of the practices described below:
☐ I consent to the collection and use of my personal information by CreditBook for the purpose of creating and managing my user account.
☐ I consent to the use of my personal information to receive service notices, administrative information or communications related to my customer relationship with CreditBook.
☐ I consent to the use of my personal information to receive promotional communications, offers or newsletters from CreditBook.
☐ I consent to receiving educational or informational communications from CreditBook concerning credit management, financial literacy or updates relating to its services.
☐ I consent to the collection and use of my personal information, including financial information and information relating to my credit file, for the following purposes:
analyzing my credit file;
assessing my financial situation;
providing advisory, support or credit-rebuilding services relating to my credit file; and
monitoring my file and communicating with me regarding these services.
☐ I consent to the disclosure of my personal information to a third-party service provider for data analysis aimed at improving our services.
☐ I consent to the disclosure of my personal information, including information relating to my credit file, to the following third parties in connection with CreditBook’s provision of services and for purposes related to the analysis, verification, correction or management of my credit file:
credit reporting agencies, such as Equifax and TransUnion;
financial institutions;
creditors; and
lenders.
☐ I consent to the transfer of my personal information outside Québec, including to servers leased by CreditBook elsewhere in Canada or in the United States, for the processing and storage of my data, including data relating to my use of our services and information relating to my credit file.
☐ I consent to the use of my personal information for analysis, the improvement of CreditBook’s services or statistical purposes.
☐ I consent to CreditBook’s collection and use of personal information relating to my credit file in order to provide me with personalized recommendations regarding my financial health, creditworthiness or credit file.
